Patients involved: 300

Patients in the intervention group are equipped with wireless blood pressure meters, wireless glucometers and wireless weight scales. Measurements are performed with a variable frequency which is indicated by the GP who normally follows the patient, based on the individual patient's needs (frequent measurements for patients in insulin therapy, less frequent measurements for patients treated with oral hypoglycaemic agents) and the level of glucose control that has been reached. The measured values are transmitted to and stored within a central monitoring unit that serves as residential or nursing home unit.

From the central monitoring unit, values are collected daily by a mobile care station or an Android based smartphone, and sent to a central health portal where the records of individual patients can be assessed by their GPs. GPs also receive an automated email summary report and GPs and nurses receive SMS alerts on abnormal values in order to trigger eventual changes in medication or insulin dose adjustments. Advice on diabetes management by changing lifestyle habits will be given by the participants' GPs when necessary.

The telemonitoring system

1. The participants are equipped with light-weight handheld devices and record their vital signs at home. Via Bluetooth the vital signs are sent to the mobile phone as a gateway.
2. The gateway sends the data via GPRS to the HIS Portal as an Healthcare Information server, the eHealth record.
3. The eHealth record handles the data and organize the alarm management
4. The responsible healthcare professionals (GP or hospital based expert or Home Care Services) access the data of the patient by using eHealth record and provides feedback.
